THE OUTREACH PROGRAMME
Community Aged Care Packages enable Holy Spirit Home to offer a range of services to a residents in their own home including Personal Care to Villa residents. The Home has a license to provide 100 Hostel places (including 2 respite) plus 5 Community Aged Care Packages.

The Department of Health and Aged Care pays the Home a subsidy to provide this care to eligible residents. Under government guidelines a contribution from the resident is also required depending on the services provided and the income of the newest resident.

ELIGIBILITY - To be eligible you will require:

	Package approval letter from the Aged Care Assessment team (ACAT) and (CACPS Co-ordinator)
	Assessment of needs by the Registered Nurse from the Holy Spirit Home

Under Commonwealth Government guidelines an agreement for this service will be required.  The Package of Services may include assistance with:

	bathing
	meals
	medications
	rehabilitation
	emotional support
	home help and other assistance

Assistance must be provided for at least 28 days.

ADDITIONAL SERVICES - Alternative Services
Residents who do not meet the criteria may still receive Community services such as Meals on Wheels, Home Help and Domiciliary Nurses (Blue Nurses) in their villa as any other resident in the community would.

Residents without Commonwealth Approval for a Community Service Package may be offered a package of care services on a fee for service basis.

Residents may also choose to privately use the catering, cleaning and laundry services offered by the Home. Arrangements for these services are made through the Housekeeper or the Business Office.
